If you would like to be referred for any of the referral services we offer, please ask your vet to complete the relevant form and send it to us with any notes, test results, radiographs. etc. We will then contact you to arrange an appointment.
We will provide an estimate of the likely costs involved. However, the final costs (particularly in complex cases) may only become apparent at the end of treatment. Usually, the initial treatment is estimated prior to surgery and a guide of what ongoing expense there may be over several weeks can be given. All accounts must be settled in full at the time of your pet’s discharge from the practice. We will, of course, be happy to complete your insurance forms for your reimbursement, if your pet is covered by medical insurance. If you would like us to claim directly from your insurers, this needs to be agreed by us in advance to allow time to speak to them. In addition, we charge a non-reclaimable fee of £16.20 for this service. You will also be required to complete an indemnity form with us in case your insurers decline payment.
Referral practices might prefer to download a referral form to print out and fax, along with the patient history, if so, please click this link:
